CEHT BenefitCare Signup
The CEHT BenefitCare program by Allan Friesen Employee Benefits is a service provided without charge to all members.
The purpose of BenefitCare is twofold. CEHT members are encouraged to submit the names of their current healthcare providers using this online form to allow us to contact those providers on your behalf to;
- confirm they have a direct billing arrangement with your new employee benefits insurer and
- if they do not currently have a direct billing relationship with your new insurer, explain the benefits to your healthcare provider and assist them in setting up this service.
By establishing a relationship with your healthcare provider, MyTeam members at Allan Friesen Employee Benefits can also assist you or your healthcare provider with questions or help resolve any billing issues should one occur.
In addition to the support described above, you have the option to sign up to receive both BenefitCare and our Community Wellness program email by simply checking the boxes at the end of this form. This service provides access to carefully vetted and curated resources and opportunities to maximize your employee benefits and take advantage of special offers and programs focused on well-being and healthcare!
Your information will not be shared with any third parties beyond the healthcare providers you list.

Please include your name, the company you work with and contact information incase we need to contact you about your providers. Then simply list your providers by practictional/professional name or buisness/ clinic name.
